No earnings ceiling: Unlike NAPSA, NHIMA has no upper contribution cap. The 1% rate applies to your full gross salary, however high it is.

How NHIMA Differs From PAYE and NAPSA

NHIMA is deducted from gross pay but, unlike NAPSA, it does not reduce your taxable income for PAYE purposes. The calculation order for a typical payslip is: gross salary, then NAPSA deducted (which lowers taxable income), then PAYE calculated on the reduced amount, then NHIMA deducted separately from gross pay.

NHIMA Calculation Formula

Employee NHIMA = Gross Monthly Salary × 1%
Employer NHIMA = Gross Monthly Salary × 1%
Total Monthly Contribution = Employee NHIMA + Employer NHIMA
Annual Contribution = Monthly Contribution × 12

Worked Example

Gross monthly salary of K10,000:
Employee NHIMA: K10,000 × 1% = K100.00
Employer NHIMA: K10,000 × 1% = K100.00
Total monthly contribution = K200.00
Total annual contribution = K2,400.00

What NHIMA Covers

NHIMA contributions fund access to healthcare services at accredited public and private health facilities across Zambia, covering outpatient and inpatient care, surgeries, maternity services, and other medical treatment for the contributing member and their registered dependants, subject to NHIMA’s benefit package and accredited facility list.

Frequently Asked Questions

Is there a maximum NHIMA contribution?

No. Unlike NAPSA, NHIMA has no contribution cap. The 1% rate is applied to your full gross salary regardless of how high it is.

Does NHIMA reduce my PAYE tax?

No. NHIMA contributions do not count as a deduction for PAYE purposes. Only approved pension contributions, such as NAPSA, reduce your taxable income.

Who registers self-employed and voluntary members?

Self-employed individuals and voluntary members register directly with NHIMA and contribute based on a declared monthly income, since there is no employer payroll to deduct contributions through.

Are dependants covered under my NHIMA contribution?

Yes. Registered dependants, such as a spouse and children, are typically covered under a contributing member’s NHIMA registration, subject to NHIMA’s registration rules.
